The Capital Health gallery can only accommodate hangable art. Artists with hangable art, please complete the entire form below and upload an image of the piece you would like to display for review by Capital Health.
If you create 3D artwork or sculpture but would still like to be recognized in the show, please complete only the required fields as indicated with a red star in the form below. You can disregard the image upload since we will use your photo and bio information from the 2024 Tour website.
Please bring the signed Capital Health display agreement when you drop off your artwork.
